Please visit the Center for Student Accessibility on the third floor of the Whitlock Building.

To register with the Center for Student Accessibility, all students must fill out an application, provide proof of their disability, and meet with a CSA staff member before services can be provided.

Disability documentation should be provided by a licensed or otherwise properly credentialed professional who has appropriate and comprehensive training, relevant experience, and no personal relationship with the individual being evaluated. Please submit documents on official letterhead which include the address, telephone number, name, title, and professional credentials of the evaluator

Documents should be signed by a certified and/or licensed professional qualified to make the diagnosis and include:

  • Clearly stated diagnosis.
  • Description of the functional limitations.
  • Current information, depending on the diagnosis, to determine the current impact. Adult scale evaluations are appreciated. This guideline may not apply to physical or sensory disabilities of a permanent or unchanging nature, depending on the request made.
  • Include educational, developmental, and medical history of the disability and accommodations requested.
  • Include a list of all test instruments used in an evaluation report and relevant subtest scores used to document the stated disability.
  • Recommend specific accommodations requested.
  • Justification of requested accommodations.

Documentation of a disability is not provided by:

 

  • Individualized Education Plans (IEPs)
  • 504 Plans
  • Transition Plans
  • Information provided on a prescription pad or handwritten document.

Providing an IEP or 504 plan from elementary or secondary school does not automatically guarantee that one will receive the requested accommodation, accommodations granted in a secondary education setting do not automatically transfer to a post-secondary education setting. However, an IEP and 504 plan demonstrate a history of a disability.

A request for an ESA also includes a verification form and current, complete vaccinations for the animal.
