Mental Health and Medical Conditions:

The Center for Student Accessibility DOES NOT accept documentation from pay-for-service websites. There are two reasons to require professional documentation under 504/ADA, (1) to establish that this is a person with a disability, and someone protected under the law, and (2) to determine what must be done to assure equal access in an educational environment. If we do not have enough information to assign accommodations, we may ask for additional documentation. The CSA must know the impact of the diagnosis on the individual (in an educational setting) to understand what needs to be provided to assure equal access.

If you are a student applying for services due to a mental health condition or a medical condition, please follow these steps. First complete our Online Application This is a simple two page application providing us with your history, and your own insights into what is most helpful for you. When you complete the application just click the submit button and your application will be automatically directed to our office. Second, begin to collect your documentation that will be required before approving your request. We have created documentation guidelines to assist you and your medical professional. Lastly, if you or your provider require a release of information, you may download the CSA Consent to Release Information form.

Request for an Emotional Support Animal or ESA: 

The Center for Student Accessibility DOES NOT accept documentation from pay-for-service websites. There are two reasons to require professional documentation under 504/ADA, (1) to establish that this is a person with a disability, and someone protected under the law, and (2) to determine what must be done to assure equal access in an educational environment. If we do not have enough information to assign accommodations, we may ask for additional documentation. The CSA must know the impact of the diagnosis on the individual (in an educational setting) to understand what needs to be provided to assure equal access.

If you are a student applying for permission to bring an ESA to campus, please follow these steps. First complete our Online Application. This is a simple two page application providing us with your history, and your own insights into why this is needed. When you complete the application just click the submit button and your application will be automatically directed to our office. Second, begin to collect your documentation that will be required before approving your request. We have created specific ESA documentation guidelines to assist you and your diagnosing professional. Your diagnosing professional will also be required to complete an animal verification form to be submitted with your documentation. If you or your provider require a release of information, you may download the CSA Consent to Release Information form. Lastly, CSA must receive the vaccination records of your ESA from your veterinarian.

As a student requesting an ESA you must familiarize yourself with the EKU Animals on Campus Policy 1.3.1. If you are approved, failure to follow this policy may result in your ESA being removed from campus.

A coordinator will contact you shortly after you have submitted the application for services. When you have gathered your documentation an intake interview will be scheduled to discuss your request, and the process for providing information to your faculty.
